New Patient Online Pre-registration (Check-In)

1. Use the link in your appointment confirmation email or text to complete Pre-registration (check-in) - you'll enter your demographic and insurance information, sign the Acknowledgement and Authorization Form (visit forms), and update your current medications.

2. Please review our Office and Financial Policies and Notice of Privacy Practices prior to signing the visit forms.

3. Complete the Health History Form, a secure online form.

New Patient Registration by Paper Forms

If you are unable to or prefer not to check-in online, please fill out the paper forms at your convenience prior to your appointment. The forms are fillable PDF files. You may complete by one of the following ways:

• Download and save the forms to your computer, then type in your response in the saved file. You may then print them out and bring to the office on your day of appointment.

• Print the forms and fill them out, then bring them to your appointment.

• If you prefer to fill out the forms in the office, please just review the forms and documents provided here in advance.

• Medical Record Request form -- you may easily access your medical records including visit notes, report of labs, imaging and pathology through the Patient Portal. However if you do not want to register for the Patient Portal or would like a copy of your medical record to be released to another party, please fill out the Medical Record Request form and submit it to our office by email, fax or in-person for processing.

The following forms or document will be given to you during your office visit, and posted to your Patient Portal under the listed section:

• Post-surgical care instructions - under My Health > Learning Material

• Colonscopy bowel prep instructions -- under My Health > Learning Material

• Referral forms -- under Health Reminder (for generic referral not to a specific provider/facility), or My Health > Referral (if you are referred to a specific provider/facility)
